Educational qualification :-

• Pharmacist : +2 Science With Diploma in Pharmacy.
• Jr. Radiographer : +2 Science With Diploma in Medical Radiation Technology.
• Jr. Laboratory Technician : +2 Science With Diploma in Medical Laboratory Technology.
• Attendant : Candidate must have passed Class- VII.

Age Limit (As on 24/10/2019) :-

• For Attendant Posts:  18 – 32 Years 
• For Other Posts:  21 – 32 Years
• Age relaxation as per rules.

Examination Fee :-

• There is No fee for this post.

Documents Required (Self Attested Photo Copy) :-

• Class-VII Pass Certificate and mark sheet for the post of Attendant, (Mark sheet should be signed by the BEO and counter signed by the DEO of the respective district).
• HSC Certificate and mark sheet.
• +2 Certificate and Mark sheet.
• Phmacist/Medical Laboratory Technology/Medical Radiation Technology qualifying examination Certificate and mark sheet.
• Registration Certificate issued by the concerned Pharmacy council/Lab.Tech. Council and RG council.
• Residential Certificate.
• Experience Certificate from the competent authority.
• Cast Certificate Issued by the competent authority in case of ST / SC / SEBC.
• No objection Certificate from the competent authority (if in Service)
• PWD Certificate Issued by the competent authority.
• Two recent pass port sixe photograph with signature.
• One envelop (214×12 cm) with postage stamp worth Rs.25/-
