Educational qualification :-
- Pharmacist : +2 Science With Diploma in Pharmacy.
- Jr. Radiographer : +2 Science With Diploma in Medical Radiation Technology.
- Jr. Laboratory Technician : +2 Science With Diploma in Medical Laboratory Technology.
- Attendant : Candidate must have passed Class- VII.

Age Limit (As on 24/10/2019) :-
- For Attendant Posts: 18 – 32 Years
- For Other Posts: 21 – 32 Years
- Age relaxation as per rules.
Examination Fee :-
- There is No fee for this post.
Documents Required (Self Attested Photo Copy) :-
- Class-VII Pass Certificate and mark sheet for the post of Attendant, (Mark sheet should be signed by the BEO and counter signed by the DEO of the respective district).
- HSC Certificate and mark sheet.
- +2 Certificate and Mark sheet.
- Phmacist/Medical Laboratory Technology/Medical Radiation Technology qualifying examination Certificate and mark sheet.
- Registration Certificate issued by the concerned Pharmacy council/Lab.Tech. Council and RG council.
- Residential Certificate.
- Experience Certificate from the competent authority.
- Cast Certificate Issued by the competent authority in case of ST / SC / SEBC.
- No objection Certificate from the competent authority (if in Service)
- PWD Certificate Issued by the competent authority.
- Two recent pass port sixe photograph with signature.
- One envelop (214×12 cm) with postage stamp worth Rs.25/-
