Full time regular ITI (2 years duration) in Electrician / Electronic Mechanic Trade. The course must be approved by National Council of Vocational Training (NCVT)/State Council of Vocational Training (SCVT) |

Senior Treatment Supervisor (STS) under RNTCP |
Bachelor’s Degree OR Recognized sanitary inspector’s course. Certificate course in computer operation (minimum 2 months). |

Pharmacist cum Logistic Assistant |
The candidate must have passed in “Degree / Diploma in Pharmacy. Minimum 01 year Experience in managing a drug store in reputed hospital/health centre recognized by the Govt. |

SSMTC under NVBDCP |
He / She should be Science Graduate in Biology/ Bio-chemistry/ Microbiology. In areas where a candidate with the desired qualification is not available, a Graduate with science subject in Class-Xl and XII will be considered. |

Laboratory Technician |
Degree / Diploma in Medical Laboratory Technology (M.L.T). Should have completed 10+2 before obtaining diploma or degree. |

Counselor cum Public Relation Officer |
Post-Graduate in Social Work / Sociology / Psychology / Anthropology / Human Development. |
|
Psychiatric Nurse |
The candidate must have passed in GNM/B.Sc. Nursing from any 3 Govt. Nursing schools of Medical colleges/ School of nursing MCL, Talcher/ IGH, Rourkela or other recognized private institutions duly approved by INC and must have registered in the Odisha Nurses & Midwives Council. S/he must have Diploma in Psychiatric Nursing. |

Must have passed + 2 Science Examination under Council of Higher Secondary Education, Odisha/equivalent & Diploma in Pharmacy from Govt. Medical College & Hospitals of the State/ any other recognized Private Institution duly approved by A.I.C.T.E. and examination conducted by Odisha Pharmacy Board. |

Have passed +2 in any stream under Council of Higher Secondary Education, Odisha or equivalent and passed Health Worker Female (Auxiliary Nurse Midwifery) Training Course conducted by the Odisha State Nursing and Midwives Board or passed out from INC (Indian Nursing Council) approved institution either Govt. or private and having Odisha Nursing & Midwives Council Registration. |
Age Limit (As on 01/02/2020) :-
- Minimum Age : 21 Years
- Maximum Age for Pharmacist & ANM Posts : 32 Years
- Maximum Age for other Posts : 40 Years
Documents Required :-
- HSC / Equivalent examination certificate and Mark sheet. (Proof of Age).
- All mark sheets & certificates in proof of the claim made by the candidate relating to his educational qualification.
- Cast Certificate for SC / ST / SEBC candidates from the competent authority.
- Valid updated Registration No of DMLT / ANM / Pharmacist etc.
- Handicapped certificate, Ex-Serviceman Certificate, Sports Person Certificate etc. if any.
- Other documents if any related to this recruitment.

Note :- The completely filled in application forms along with other document should reach the office of the Chief District Medical Officer, Cuttack (At- Old Secretariat Campus, Cantonment Road, Po- Buxi Bazar, Dist- Cuttack-753001) on or before 24.02.2020 till 5 PM through Speed Post / Registered Post only and the application should be Super-scribed with the name of the post applied for otherwise it will be rejected.
